This document concerns the services selected below on the appointment date entered below. Please read it carefully. It includes a release of certain legal claims. Ask questions before signing. This form is for clients age 18 or older; minors require a separate consent process.

Services for this appointment

1. Voluntary consent

I voluntarily request only the services selected above. Before treatment, I will discuss the service, products, relevant risks, alternatives (including no service), preparation, aftercare, and any questions with my artist. I may decline a service or ask the artist to stop. My signature does not authorize additional or undisclosed procedures.

2. Risks and individual results

I understand cosmetic services involve risks even when appropriate care is used. Depending on the service, these may include discomfort, redness, itching, irritation, swelling, allergic reactions, sensitivity to products or adhesives, skin lifting or abrasions, burns, infection, changes in skin color, hair or lash breakage or loss, and eye irritation or injury. Reactions may occur during or after an appointment. Serious reactions can require medical care.

Results and longevity vary. A patch test, when recommended or required for the product and service, does not guarantee that a later reaction will not occur. I understand that no particular appearance, duration, or outcome has been guaranteed.

3. My disclosures and participation

Before service, I will accurately disclose relevant allergies, past reactions, skin or eye conditions, current irritation or infection, recent procedures, and medications or skincare products that could affect treatment. I will discuss any other relevant health circumstances privately with the artist and update the artist if anything has changed. This public website is not the place to submit sensitive health details.

I will follow the preparation and aftercare instructions explained to me, promptly tell the artist about discomfort or a reaction, and seek appropriate medical attention for concerning symptoms. The artist may postpone or decline a service if it appears unsuitable. This form does not make an unsuitable service safe or authorize work beyond the artist’s qualifications or lawful scope of practice.

4. Assumption of disclosed risks

After discussing the selected services and having an opportunity to ask questions, I knowingly and voluntarily accept the inherent risks of those services that have been disclosed to me and that can occur despite reasonable care. This acknowledgment does not excuse conduct that cannot lawfully be released.

5. Release of claims for ordinary negligence

To the fullest extent permitted by applicable law, I release Vanice Allure, Keisha, and the studio’s employees or service providers who perform my selected services from claims for personal injury or loss arising directly from those selected services at this appointment, including claims caused by their ordinary negligence.

I understand that “ordinary negligence” means a failure to use reasonable care, and that this paragraph asks me to give up the right to recover for certain injuries or losses caused by such a failure.

This release does not cover gross negligence, reckless or intentional misconduct, fraud, violations of law that cannot be waived, undisclosed or unauthorized procedures, or any rights or liabilities that applicable law does not permit me to waive. It does not apply to unrelated incidents or future appointments.

6. Important limits

This form does not waive the studio’s legal duties that cannot be waived, authorize unlawful services, prevent me from reporting a concern to a regulator, or require me to pay another party’s legal costs. It does not authorize photographs, marketing, or disclosure of my private information. Any photo permission must be obtained separately.

If part of this document is unenforceable, the remaining provisions apply only to the extent permitted by law. Nothing in this form guarantees that the release will be enforceable. Any separate appointment payment or cancellation terms remain subject to applicable law and are not expanded by this release.

7. Acknowledgment and signatures

I am at least 18 years old and am signing for myself. I have read this document, understand its release language, and had an opportunity to ask questions before signing. I sign voluntarily and understand that this form applies only to the services and appointment identified above.
